Mr. Malik strives to provide you with the best care possible. Your first visit is an important step towards understanding your health needs and developing a personalised treatment plan. Here's what you need to know for your first appointment:
Please make every effort to arrive on time for your appointment. If you anticipate being late, kindly notify Mr. Malik's secretary or the clinic as soon as possible. Your punctuality helps us maintain a smooth schedule for all patients.
Insurance Information: If you have private medical insurance, please bring along your membership number and authorisation code.
Referral Letter: If required.
Medical Records: Any relevant documents such as results, X-rays, MRIs, CT scans, or other diagnostic reports.
Medical History: Your past medical and surgical history, as well as a list of current medications.
Medical History and Examination: Mr. Malik will take a comprehensive medical history from you followed by a clinical examination.
Further Investigations: Based on your assessment, additional tests such as X-rays or blood tests may be necessary. If needed, you may be referred for outpatient scans like CT, MRI, or ultrasound.
Should you need to cancel your appointment, please contact our office during business hours.
We kindly ask for at least 2 days' notice to accommodate other patients on our waiting list.
